Broken Heart EP2 Pain

"Scalpel"

The nurse hands me the scalpel.

"Oh, Mercedes, is it?"

Professor Jyomi sees the movement of the scalpel and says.

"Yes, considering the location of the arrow and the treatment of the liver."

"Is that so, very well."

Professor Jyoumi says to himself.

"Suction, please."

"Yes." Rituko sucks up the blood that has accumulated.

"Saline, please."

"Yes."

She washes the wound. Rituko widens the surgical field while continuing to suction.

The CT image shows the arrow stuck in the head.

She widens the surgical field further and checks the position of the liver and the arrow.

Fortunately, the arrow pierced the left lobe. The proportion of blood vessels is smaller than in the right lobe. But if you pull out the arrow carelessly, you can't avoid massive bleeding.

She carefully opens the part where the arrow is stuck and ties off nearby blood vessels.

"Monopolar"

A faint white smoke rises.

"Metzenbaum"

"Satenski" She ratchets it.

She checks the condition of the liver and looks at the transverse colon.

"But it's amazing how it avoided the gallbladder and stomach."

Indeed, normally if an arrow is stabbed from this position, the liver and stomach are definitely damaged. But this time, for some reason, the arrow pushed in the stomach and reached the transverse colon.

"I wonder what posture it was stabbed in. More importantly, it was lucky that it didn't go to the aorta."

"Yes, indeed. And it's also the left lobe of the liver. This kid might be a lucky one. He's been living a normal life with congenital analgesia."

"It's miraculous."

Rituko says casually.

She cuts off the damaged part of the transverse colon.

"Metzenbaum"

"Monopolar"

"Then I'll pull it out."

"Gauze"

She protects other parts from being hurt.

She slowly and carefully pulls out the arrow.

The arrow moves little by little. Slowly and carefully, the arrow leaves the body.

After leaving the liver, blood gushed out from that part.

"Blood pressure dropped to 70"

She doesn't panic. This was what she expected.

Rituko quickly sucks up the blood that overflows.

Professor Jyomi just watches and doesn't try to help.

"Satenski, blade"

To stop the blood flow from the bleeding part, she temporarily ties off a blood vessel.

She repairs the liver damage.

"Monopolar, mosquito, 3-0 polypropylene. Cooper"

The nurse who does instrument delivery selects and delivers them quickly and accurately to the surgeon.

She sutures and connects each major blood vessel one by one.

She repairs the damaged left lobe of the liver.

She releases the tied blood flow.

The liver that had turned black gradually regained its original color.

No leakage was confirmed from the anastomosed blood vessels.

"Blood pressure recovered"

"Then we'll move on to repairing the transverse colon."

She pulls out some of the intestines from inside and cuts off and reconnects

the damaged part to rebuild it.

Basic surgery is cutting and sewing. It's a continuous process.

But there are various methods for that technique in surgery. You need to perform

the appropriate technique according to each part and condition.

To judge that and do it reliably and quickly and accurately, you have to gain experience.

Surgeons have to polish their experience and skills.

She checks again on

the repaired part. There seems to be no problem.

"Can I check it?"

Professor Jyoumi looks into

the surgical field.

"Yeah, it looks stable and there's no problem. Dr. Tanabe"

Without looking away from his stern eyes,

she said "Thank you".

"Then let's close it up. V-Loc PBT"

Rituko secures

the surgical field while closing

the wound with continuous sutures.

Finally, she cuts

the thread and

the operation is over.

"Vital signs, blood pressure 100 over 70, heart rate 90 stable."

"Good job, Dr. Tanabe"

Smiling as if ... but I can't tell for sure because of

the mask,

she heard Rituko's voice,

and finally her tension was released.

"Thank you." She said one word and left

the operating room.

She took off her gloves, took off her surgical gown, and headed for Yuuma-kun's parents.

After the operation, I have to report the progress to the patient's relatives. It's called Muntela. And I have to explain the treatment for the future.

In Yuma-kun's case, there was no special disease in the current situation.

But the symptom of congenital analgesia is a special matter. I can't predict the changes of symptoms and so on from now on.

Because he doesn't feel pain or suffering.

In front of the operating room, Yuma-kun's parents were sitting exhausted on the chairs in the corridor.

When the door opened and they saw me, they stood up with a I'm surprised and ran over to me.

It's no wonder. Their child was seriously injured and underwent surgery for a long time. During that time, they were full of anxiety.

"Doctor, Yuma is..."

The mother says in a voice that seems to cry at any moment.

"Yeah, Yuma-kun did well. I was able to repair the damaged part. I don't think there's a problem."

Hearing that, the mother let out all the tears she had been holding back.

I moved my eyes to the father.

"But the problem is the congenital analgesia that Yuma-kun has. Regarding this, I don't know until I see how it goes from now on. How it will appear in Yuma-kun, I still don't know."

"So, but Yuma is fine now, right?"

The father asks back.

"Yes, he will be moved to ICU soon. He will regain consciousness soon. Please be with him then."

"Thank you."

The father bowed his head deeply and thanked me.

The bed with Yuma-kun on it came out of the operating room. They both clung to the bed as they went to ICU together.

As I watched their parents, I suddenly thought in my head.... The smile that my mother showed me at the end came to mind.

I guess I can't erase that smile of my mother.

Maybe thanks to that.

I met Mayumi because of that smile of my mother.

But they left me with the same smile and disappeared.

And another smile, I deliberately sealed it inside me. If I hadn't done that, this me wouldn't exist here now.

Once, I tried to run away from this world, because it was true....

The person who saved me from losing everything as a doctor and as a human being was Professor Jyoumi.

At that time, he was an associate professor at Hokubu Medical University, Jyoumi Kousaburou.

He transferred me to a city hospital affiliated with him as a part-time doctor (part-time job) for a few months after I submitted my resignation.

At that time, he called me to his place and gave me the notebook that Mayumi left for me along with it.

Thanks to those few months, I can exist as a doctor and as a surgeon again.

People can grow because they understand the reason for their pain by knowing pain.

If you get hurt, you feel pain from that part. Actually, the brain sends out the pain signal, but by feeling pain, you can take action to protect that part.

But what if you don't feel this pain?

No matter how fatal the injury is, you can't do anything about it.

You might not be able to protect yourself by yourself.

That might be true for wounds of the heart as well.

To know pain is the same as to know yourself.

I think.

◆Part-time Doctor EP0

"Tanabe, have you finished sorting out the charts?"

"I'm sorry, I haven't done it yet."

"What do you mean? How long are you going to take!"

"I'm sorry."

I had been transferred to this hospital as a part-time doctor for a few days.

This hospital is not well-equipped like a university hospital. Cutting-edge medical devices? There's nothing like that in this hospital no matter how much you look for it.

Medical facilities? Sure, there's a CT. But that's also an old-fashioned one. Honestly, I'm surprised they're still using this thing.

Blood tests are also very limited in what they can do in-house. They can only do basic items. For other items, they have to outsource them all.

The number of beds is 60. The medical specialties are basic internal medicine and surgery. But even if patients come to the outpatient clinic with other diseases, they don't care. It's like an all-purpose diagnosis.

And on top of that, the facilities are poor and there are few doctors and nurses.

Not to mention the building itself is very old... No, I should say it's doing its best. I'm impressed it's holding up.

When I first came to this hospital, I hesitated to step into it.

"Nice to meet you, Dr. Tanabe. I'm Matsumura, the director of this hospital. Nice to meet you."

The director of this hospital "Matsumura Tadashi" has a soft expression and a very gentle demeanor.

"You don't have to be so nervous," he said, caring for me as his appearance suggested.

"Are you the doctor who was recommended by Professor Jyoumi? You're a surgeon, huh? You worked in emergency, right? That's amazing. You must be a very excellent doctor. Professor Jyoumi recommended you to our hospital, after all."

"Ah, well, I don't know if I'm that excellent or not..."

"No, no, Professor Jyoumi and I were classmates in medical school. We still drink together often."

"Is, is that so?"

"He's an associate professor at the university hospital, and I'm just a humble director of this city hospital. There's a world of difference between us, haha."

Dr. Matsumura laughs cheerfully.

But he's a really kind-hearted doctor. And even though he's the director of this hospital, which is not very large, he talks to me casually.

"Oh, by the way. When you came to this hospital, did you hesitate for a moment? Or rather,"

He smiles slyly.

"Did you think you made a mistake?" he said.

I had no words to say. Honestly, his words were right.

"Dr. Tanabe, this hospital is different from the university hospital you were at. The facilities are poor and the number of doctors is small. That's why this hospital is run by the manpower of doctors and nurses."

"Manpower?"

"Yes, manpower. But not the number of people, but the skill! Don't get me wrong."

He smiles meaningfully again.

I thought then, did I come to a terrible place...?
